Funders
The John A. Hartford Foundation
TFAH is proudly supported by The John A. Hartford Foundation, a private, nonpartisan, national philanthropy dedicated to improving the care of older adults. The leader in the field of aging and health, the Foundation has three priority areas: creating age-friendly health systems, supporting family caregivers, and improving serious illness and end-of-life care.

Michigan Health Endowment Fund
In partnership with the Michigan Public Health Institute, and with funding from the Michigan Health Endowment Fund, TFAH is supporting a Michigan AFPHS initiative focused on building partnerships between public health and aging services to enhance and support older adult health and wellbeing in Michigan.

Cambia Health Foundation
TFAH’s work to ensure access to the COVID-19 vaccine for homebound older adults is sponsored in part by the Cambia Health Foundation, the corporate foundation of Cambia Health Solutions, a total health solutions company dedicated to transforming the way people experience health care. Cambia Health Foundation makes purposeful philanthropic investments that transform health care, making the journey more person-focused and economically sustainable from birth to natural completion of life.
