Descriptions of activities aligned with the 6Cs: Departments of health seeking recognition must submit an action plan that includes (at least) one activity for each one of the 6C tenets. This list of activities is meant to provide guidance. Departments can select among this list of activities that are outlined according the 6Cs Framework or identify activities that better meet the unique needs of their older adult population. This resource also provides links to examples from other states. A more extensive list of activities can be found here.

Action Plan Template: Departments of health will be designated as “AFPHS Recognized” by submitting a completed action plan that includes (at least) one activity for each of the 6C tenets. A department that completes these activities within two years will be designated as AFPHS Advanced.
