Eligibility: All state, territorial, local and tribal public health departments are eligible to participate in the Recognition Program.

Health departments that desire to enroll in the Program should send an email to afphs@tfah.org and designate a point of contact.

TFAH will reply with a link to the Overview Webinar, the Guidance resource, and the Quarterly Report Form.

Participants should send the Quarterly Report Form with updates on the 10 action steps.

Once all action steps have been achieved, participant should notify TFAH and will then receive the Recognition Certificate and AFPHS Recognition badge.
